Meaningful Use/ Menu set, Measure 4: Patient Reminders

Objective: Send Reminders to Patients per patient preference for
preventive/follow up care

 

 

Dear PCM Client, 

 

In preparation for 2014 Meaningful Use, all practices need to start looking ahead to be sure that they are using the automation built into PCM for properly populating the Meaningful Use data table as there will be a requirement that no manual updating is allowed.

 

Effective next PCM update, to protect users against audits, we are starting to enforce Measure 4. In order to qualify for Measure 4, the Meaningful Use table must contain the document ID (DOCID) for the reminder that was sent.

 

The Meaningful Use Measure Details will be changed to:

 

"Numerator: Patients from denominator that have a record in their Meaningful Use data table for the Event Code of '5 Patient Reminder Sent'. The record is added to the Meaningful Use data table when a user generates the reminder using the patient reminder log. It can be manually updated as well, but for reporting 2013 and beyond, the data table must also have the DOCID column, as when reminders are generated, the proof that reminder was generated is in that field not being blank. If patients do not have their preferences set in the demo editor, then it will not count either."

Important Notes:  

 

  • When generating Patient Reminders from the Reminders screen, the document ID always gets updated.

  • Manually updating the Meaningful Use table with Event Code '5 Patient Reminder Sent' and NOT updating the document ID will no longer count towards Meaningful Use.

  • Copies of all reminders sent are stored in the chart
