Utilization by Age and Gender (A), Utilization by Procedure Category (C), Utilization by Diagnosis Group (D), Utilization by Poverty Level (P), and Utilization by Race (R). |

Report Option |
Required for all Report Codes This field determines whether the three sub reports are summarized by Clinic or Provider. |

Provider |
Used for all Report Codes The default is all providers (leave blank). To filter the results based on a specific provider, type the Utility --► Provider <Provider Code>.Note: This field does NOT accept multiple values, but accepts an asterisk * to return all Providers starting with the portion of the code entered prior to the *. |

From Date / To Date (optional) |
Used for all Report Codes These dates are not required. Leaving it blank causes Intellect to search all records up until the <To Date>. Type the first and last entry dates for the data to include in the report. |

Date Selection |
Used for all Report Codes This field defaults to the Entry Date. Use the drop-down to make an alternate selection. |
