Company Statement 7 is a custom statement and looks similar to 5 and 6.

Company Statement 7 works similar to a guarantor/child statement. The employer is set up like a guarantor. The employees have their own accounts as like a minor child. As in a guarantor set up, statements are printed to the employer (guarantor) account number.
DATE: /New Patient <Patient’s Account Number> and date of service
PATIENT NAME/SS OR ID: /New Patient <Name (Last, First Init.)> / <Social Security No.>
DR.: /Utility/Provider <Provider Code> used in posting the charge
DESCRIPTION: /New Patient <Employer> and /Utility/Procedure <Description>
CHARGE: Charge amount at time of posting
CREDITS: Combined payment and adjustment amount.

Guarantor: Use ONLY option A or Z.
A Will copy the guarantor information during the process of adding a new patient which would then need to be modified to the patient’s information.
A Will allow this feature use in true guarantor situations.
Z Will not copy the guarantor information during the add process.
Z Will not allow this feature to be used in true guarantor situations.

NOTE: When more than one clinic is to print Company Statement 7, add additional CS CODE’s. For example, CS2 for clinic 2, CS3 for clinic 3, etc. making sure TYPE is blank, Seq. is 69 and DEFINITION is CS1.PRN.
/Utility/Category
It is suggested to add a COMP (company) category to be used for the employer accounts. Then, for each employer, add a separate category code, for example EDIS (Edison Co.), PCHM (PharmChem), SEAR (Sears), etc. What the category codes are is up to your discretion.
Since charges are posted to the patient account and management reports may be ran for specific categories, using this theme will provide detail financial information by employer.
In addition, unlike a typical guarantor setup, the employer and patient must have different categories in order to assign the required category type to each account.
Type (C/P/W/O/F/G): Enter C on the employer’s category
Enter O or G on the patient’s category
Guarantor: Repeat the employer account number on both the employer and patient’s accounts.
Category: For the employer’s account, enter the category used for company accounts.
For the patient’s accounts, enter the category used to denote that patient’s employer.
/Charges/Charge Post charges to the patient’s account as usual.
