Add County Code to Zip

Note: It is not required to add the county code to the zip code after July 1, 2017.

 

When Phase 2 of the CHDP Transition to National Standards is effective for dates of service ON or AFTER July 1, 2017, the PM 160 will no longer be used for submitting CHDP claims. This documentation is only applicable for CHDP set up and charge posting for dates of service PRIOR to July 1, 2017. Please refer to Charges --►CHDP--►Transition to National Standards documentation for posting dates of service on or after July 1, 2017.

For CHDP forms, the county of residence is required in two places on the form. Note: Pasadena, Long Beach, and Berkeley require a city code.

 

 

To add the county and county code to the Zip Code record:

 

1. Place the focus in the patient's <Zip Code> field and press the [Space Bar] to clear the field.
Note: The steps below can be completed from any <Zip Code> field on any screen in Intellect.  

 

2. Press the [F2] key to open the Searching screen:

 

 

3. Enter the zip code, city, or state in the appropriate field on the Searching CITY_STATE window, then press [F2] to begin the search.

 

4. Use the scroll bar to scroll through the list of zip codes and click on the applicable code. Then, click the [Modify] button on the Searching screen to open the Modify CITY_STATE window:

 

 

5. <County>: Type the name of the county. 'County' does NOT need to be included as part of the name.  

 

6. <County Code>: Enter the 2-digit code assigned to the county by the State of California.
Note: Pasadena, Long Beach, and Berkeley require a city code.

 

 

7. Click on the [Modify] button.
