What About MU Stage 2?

We are anticipating that Prime Clinical and PCM will be registered for MU Stage 2 by June 2014. This means you will have until the last quarter of 2014 to be able to report for the 3-month EHR reporting period.

3-Month Reporting Period for 2014

For 2014 only, all providers, regardless of their stage of meaningful use, are only required to demonstrate meaningful use for a 3-month EHR reporting period. For Medicare providers, this 3-month reporting period is fixed to the quarter of calendar (for EPs) year in order to align with existing CMS quality measurement programs, such as the Physician Quality Reporting System.

CMS is permitting this one-time 3-month reporting period in 2014 only so that all providers who must upgrade to 2014 Certified EHR Technology will have adequate time to implement their new Certified EHR systems. Visit the CMS website for more information.

Use CMS' EHR Participation Timeline

Determine and calculate your incentive payments for both Medicare and Medicaid using CMS' EHR participation timeline posted on our website.

The timeline also assists in determining if you will be attesting for Stage 1 or Stage 2 Meaningful Use in 2014.

EHR Participation TimelineAccess the timeline button by going to www.primeclinical.com, logging-in with your Client ID and password, selecting [Meaningful Use Tools], then [Learning Tools], then [Medicare & Medicaid Information], and then 'Getting Started'. Scroll down to the timeline button located mid-page, and click where indicated.

New HCFA 1500
Changes Affect Unix Clients

A new Health Insurance Claim Form (HCFA) has been released, which goes into effect January 6, 2014.

Some of the changes to be aware of include:

Use commas to separate the last name, first name, and middle initial. A hyphen can be used for hyphenated names. Do not use periods within the name.

8-digit dates must be used in all date-of-birth fields (items 3, 9b, and 11a)

 Leave Box 8 blank

Note: The new HCFA 1500 changes will not be available on the OnStaff Unix systems. Clients on Unix will need to upgrade to the Windows version of OnStaff.

See additional information for individual boxes on the form.

ICD-10

Anticipated Operational Impact
of ICD-10

Experts project up to a 40 percent decrease in coder productivity during the first year of implementation.

In such a scenario:

A facility with seven inpatient coders could potentially experience a $1.4 million daily impact

A facility with six outpatient coders could anticipate a 20 percent productivity decrease in the first year for a potential $25,000 daily impact

When Gayl Kirkpatrick, 3M coding services manager, polled a Sept. 24 webinar audience about ICD-10 impact on coder and CDI productivity, accurate documentation concerns topped the list at 81 percent. More than half expect a 30 to 50 percent reduction in CDI productivity and plan to add staff. Sixty-three percent expect diminished coding productivity to add days to bill drop goals.

How does your office compare in ICD-10 preparation?

Current coding audits, supplemental coding, and CDI services are a few things organizations can do now and during implementation to minimize negative impact.

Top Concerns about ICD-10 Implementation
and Approach to Training

In her Aug. 6 webinar on ICD-10 education, 3M expert Sue Belley found that 43 percent of attendees were currently using hosted, web-based training, and 50 percent were including basic awareness for staff. Top ICD-10 implementation concerns continue to be physician training (79 percent) and improving clinical documentation (75 percent).

See survey results.

Physician Assistants Can Help
With ICD-10 Implementation

Like it or not, modern medicine is increasingly becoming a data-driven machine. Documentation is everything in ensuring that we get reimbursed appropriately for the work that we do. This significant shift in the practice of medicine falls especially hard on the practitioners who graduated more than 20 years ago.

I have been a strong promoter of the physician-physician assistant (PA) team in my blogs. One interesting statistic is that nearly one-half of the currently practicing PA profession has graduated from training in the last five years. This alone ensures that these healthcare providers and team members have a strong grounding in computer skills as well as EHR use. This is the future of our professions, and in the interim, including a PA in your team will help ease the transition to ICD-10, and full implementation of the EHR.

CMS Director, Matthew Albright:
Let's Talk ICD-10

Based on feedback from medical and trade associations and other stakeholders, we have developed a variety of ICD-10 resources for Providers, Payers, and Vendors. These resources cover topics ranging from a basic introduction to ICD-10 to Continuing Medical Education/Continuing Education courses with a roadmap and guide for small practices. For a more in-depth explanation of how to guide a practice, hospital, or payer organization through the ICD-10 transition, CMS has created the Online ICD-10 Guide, which can be found on the Provider Resources page. The ICD-10 website also offers checklists and timelines, as well as FAQs, guides, and tips geared toward various audiences. CMS also attends conferences and hosts online events, to educate and encourage providers to transition to ICD-10.

Our health care partner associations also offer resources and trainings on ICD-10 designed specifically for their members. Check the Provider Resources page for a list of some partner associations that offer ICD-10 resources.

With less than one year to go before the October 1, 2014, transition date, now is the time to talk to others and make progress on ICD-10.
